1. General information concerning the application for clearance
a. Application is voluntary. However, refusal to provide any information can cause serious delays in granting access approval.
b. A Special Background Investigation (SBI) and a polygraph interview are required to assist DOD in the determination of access eligibility. If you have a current SBI and polygraph (within five years), either as a DOD employee or DOD contractor, you may not be required to be re-polygraphed.
c. All information concerning the application is confidential between the Onyx Point, Inc. Security Office, the US Government and yourself.

2. Basic criteria for DOD SCI access eligibility:
a. You must be a U.S. Citizen.
b. You must be stable, of excellent character, and of unquestionable loyalty to the United States.
c. Members of your immediate family must be United States citizens. Immediate family includes your spouse, co-habitants, children, parents, parent-in-laws, roommate(s), half and step relations, and siblings.
d. Members of your immediate family and close personal associates must be of unquestionable loyalty to the United States.
e. You, members of your family, or close associates shall not by reason of allegiance or personal situation be subject to subversive influence by a foreign power.

3. Occurrences to report on your security paperwork
a. Any arrest regardless of the outcome of the incident or when it occurred.
b. Anytime you have been arrested, charged, cited, held or detained by any law enforcement agency or juvenile authority regardless of whether the charge was dropped, dismissed or you were not found guilty.
c. Any treatment or counseling by any health professional.
d. All drug use, even on a one-time basis.
e. All use of alcohol that resulted in disciplinary action, arrest, detainment, and/or alcohol related treatment or counseling.
f. Negative details of your credit history such as bankruptcy, garnishment of wages, tax liens, unpaid judgments and delinquent debts.
g. All residences, education, and periods of employment and unemployment during the past ten years, or to your 18^th birthday; which ever is shorter.

4. General Information concerning the polygraph interview
a. A Department of Defense certified examiner would conduct the polygraph examination. Prior to the test, the examiner will review the SF86, the supplemental drug form, and the questions to be asked during the polygraph interview, as well as providing an explanation of the polygraph instrument and its function. Any problems or questions concerning the polygraph procedure or polygraph questions should be resolved with the examiner at this time.
b. Any routinely prescribed medication should continue to be taken.
c. If you have a respiratory or heart condition that is under the care of a physician, it may be advisable to obtain a medical wavier from your physician in order to continue the polygraph examination.
d. Nervousness at the time of the examination is expected, but will normally have no bearing on the examination results.

5. The polygraph interview will cover the following items:
a. Deliberate falsification of the form SF86.
b. Deliberate divulgence of classified information to unauthorized persons(s).
c. Involvement in espionage, sabotage, subversion, terrorism, or any activity in support of the overthrow of the Government of the United States or the use of force or violence to deny individual constitutional rights.
d. Secret contact with foreign government or agencies.
e. Involvement in serious crime.
f. Illegal or improper use of drugs, narcotics or other controlled substances. Statements made on the Supplemental Drug Form will be subject to specific questions during the polygraph examination. The signing of this form at the time of the submission is an acknowledgement of the agreement to abstain from improper use of drugs as of the date the form is signed.
g. Financial Delinquencies, to include bankruptcy.
h. Serious mental or personal problems.

6. After DOD SCI access is approved and you have been indoctrinated, the following restrictions apply:
a. All foreign travel must be reported to DOD forty-five days in advance of travel.
b. All unofficial travel to communist controlled countries or other hazardous areas must be approved prior to travel and a defensive briefing will be given to you either by DOD security or the Onyx Point, Inc. Security Office.
c. Resumes and professional articles submitted for publication in professional magazines and books, correspondence with foreign professionals, and speeches and papers given at professional meetings will be submitted to DOD through the Onyx Point, Inc. Security Office.
